The label says most people bleed more heavily than they do during a heavy menstrual period. Abdominal pain and cramping are expected. This is not a heavy period.

Cramping can be severe — worse than a period. That does not make it labor as a medical term, and it does not cancel the ER list on this page.

The Medication Guide says the pregnancy is likely to be passed from the uterus within 2 to 24 hours after taking both medicines. Bleeding or spotting then lasts an average of 9 to 16 days. Hours for the pass. Days for the bleed. That is the label, not a schedule for you to follow. Those hours-to-days figures are after both medicines, not after the first pill alone.

Nausea, chills, dizziness, vomiting, headache, and diarrhea are among the common reactions on the label. Weakness too. About 85% report at least one adverse reaction. Those figures are for the approved two-medicine regimen.

Bleeding and cramping can look like a miscarriage. They can also be a ruptured ectopic. They are not “just a miscarriage.”

If you are unsure whether this is expected or an emergency, it is an emergency until someone can tell you it is not. Go to an ER in Birmingham, Mobile, Huntsville, Montgomery, or the closest hospital with an emergency department.

Soak through two thick full-size sanitary pads per hour for two consecutive hours: that is an ER. If you are crashing before the second hour, do not wait. Fever of 100.4°F or higher lasting more than 4 hours belongs there too. Worsening severe pain. Fainting. Tell the ER you took the abortion pill — mifepristone, misoprostol, or both, if you know.

Serious and sometimes fatal infections and bleeding occur very rarely. No causal relationship between mifepristone and misoprostol and those events has been established. Prolonged heavy bleeding may be a sign of incomplete abortion. Serious infection can present without fever.

The approved regimen is not treatment for ectopic pregnancy. A ruptured ectopic can be fatal. One-sided pain or pain at the tip of your shoulder is an ectopic concern. Treat it as an ER.

FDA labeling says follow-up about 7 to 14 days after mifepristone is very important. If listed contacts cannot be reached, go to an ER.

Fainting is an ER. Ordinary dizziness is expected. If you cannot stay upright when you stand, treat that like fainting and call 911.
